This form is intended for members to report house incidents to central leadership. Incidents include things like property theft, noise violation, personal injury during event, etc.
We will try to uphold the level of confidentiality you select though a duty to warn may apply: if a threat of physical violence against a reasonably identifiable third person and the recipient has the apparent intent and ability to carry out that threat in the foreseeable future, the mental health professional has a duty to take action.

This is the form filled out by a President, Vice-president, or DART Chair to receive a scholarship.
The President may apply for the scholarship by filling out an application, available from the Coordinating Committee and returning the completed application to Coordinating Committee. The President cannot be turned down for the scholarship the first time they apply, but they must fill out the application if they wish to receive the scholarship.
A. The Coordinating Committee may choose not to allow the President to apply for the scholarship if they have not been trained. This choice must be reported to the Board.
Each Vice President may apply for the scholarship by obtaining the application from the President and returning it to the President. Each Vice President cannot be turned down for the scholarship the first time they apply, but they must fill out the application if they wish to receive the scholarship.

A DART Chair may apply for the scholarship by filling out this application, available from the President and returning the completed application to the president. A DART Chair cannot be turned down for the scholarship the first time they apply, but they must fill out the application if they wish to receive the scholarship.
If DART is co-chaired, the chairs may split the scholarship as appropriate. The cochairs must present the Board with a proposal to split the scholarship. In the absence of such a proposal, the scholarship shall be split evenly between the cochairs. The proposal must be approved by 85%.
The MAC may apply for the scholarship by filling out an application, available from the ICC President, and returning the completed application to the ICC President. The MAC cannot be turned down for the scholarship the first time they apply, but they must fill out the application if they wish to receive the scholarship.
This form is filled out by a member who does not have any future contracts with the ICC. This is not for ICPSR, Global Reach, CUFE, or CUEB members.
Shares are returned twice a year based on the last contract period during which the member had a contract.
Fall/Winter shares are returned by the end of the following October and Spring/Summer shares are returned by the end of the following March.
Please keep in mind we are a cooperative and delays may occur due to labor shortages and staffing changes.

THIS QUESTION IS NOT REQUIRED. Standing Rule Chaper 2 outlines four criteria for recieving a share return outside of the standard two times a year.
We maintain a database of former co-opers. We use this information for our mailing list which is for ICC events, the Alumni Cooperator, and house specific events or reunions. We will never sell or give your information to anyone outside of ICC members, alumni/ae, or staff. Let us know how you want to continue your involvement with us and the cooperative movement.
