Standing Rules 12.6 (Party Policy) and 2.5 (Controlled & Illegal Substance Policy) provide our members with further guidelines and support. Please provide the information requested below for your checklist.
SR 12.6.3
How are you going to respond to potential emergencies?
community resources, police, campus safety, ICC President, etc.
A. Be on the premises throughout the event.
B. Remain sober at all times.
C. Ensure the event's compliance with the House Party Plan.
D. Speak with any and all authorities in the event of an emergency.
E. Prepare written reports of any incident occurring during the event that involves violence of any kind, illness, injury, or harassment of any kind.
F. Submit all Incident Reports to the ICC Office within 24 hours of the incident regardless of emergency status. (link to web incident report)

This is the form you fill out when you want to leave your contract before the official end date.

If you do not have a replacement selected the ICC can assist in advertising in addition to your own efforts.
A member can be replaced by either direct recruitment (suitable replacement) or by any member once all open and uncontracted spaces have been filled. Although strongly encouraged, replacement members do not have to sign for the same house or room, except for King House.
Suitable Replacement: A suitable replacement is defined as either:
1. A brand new member with no prior contact with the ICC and who has not submitted a membership application.
2. Any person who does not have a contract for the term in question, only if the house is full.

I understand that by submitting this form, I acknowledge I am attempting to be released from my contract.
A. My current contract remains in effect until I have been officially released by the ICC. I remain responsible for all house and ICC charges.
B. I understand that I will be notified by email if I am replaced. Verbal notification of my replacement is neither sufficient nor final.
C. If there is a hold on my space by someone wishing to replace me, I may not rescind this replacement request until the hold date has expired.
D. By submitting this Replacement Request Form I am relinquishing my seniority rights (or rights to a specific room) for the contract period in question starting from the date signed above, unless my house constitution reads otherwise.
E. If I am released from my contract, I must relinquish my space.
F. Submitting this form entitles the ICC to advertise my space and replace me if possible, provided I have not indicated I will directly recruit my own replacement.
